On May 21, traffic investigators in West Java, Indonesia, concluded that negligence by a Green SM taxi driver was the primary cause of the deadly railway crossing accident involving a commuter train in East Bekasi last April.
According to Bekasi Metro Police traffic chief Commissioner Gefri Agitia, the case is being handled under a police report filed on April 27, 2026. Investigators stated that the taxi was under registration number B-2864-SBX and driven by a man identified as RR. He was travelling from Duren Jaya towards Juanda street before the collision occurred.
Authorities said the vehicle reportedly stalled on the first railway track at a level crossing in Bekasi Timur after experiencing an electrical system failure. Moments later, a commuter train travelling from west to east struck the stranded car.
The taxi sustained heavy damage, while the incident triggered a wider rail disaster. Authorities said the initial collision forced a commuter train bound for Cikarang to make an emergency stop at Bekasi Timur Station. While stationary, the train was then hit from behind by the Argo Bromo Anggrek intercity service, worsening the tragedy. The crash, which took place on the night of April 27, left 16 people dead and dozens more injured.
Investigators from the Bekasi Metro Police traffic unit have examined the crash site, reconstructed the sequence of events and questioned several witnesses, including the railway crossing gatekeeper, the taxi driver, the commuter train driver and expert witnesses.
Moreover, authorities added that the taxi driver could face charges under Article 310 paragraph (1) of Indonesia’s 2009 Traffic and Road Transportation Law concerning negligence in traffic accidents.
